I'm getting the error: "AppClosed. AknIconServer KERN-EXEC 3" when trying to use AknIconUtils::SetSize in 3rd edition.
The weird thing is that in this container I'm using other images which are loaded from the same .mif file which can be sized and displayed correctly. The images causing the error seem to be loaded correctly from AknIconUtils::CreateIconL.
Any suggestions?

"Seem" is not enough. The icon is not created. Check the indexes you use with the content of the mbg generated file.
Its ok, the problem was actually nothing to do with the code but instead the svg images themselves.
We'd been creating .svg files using adobe illustrator, then using the nokia tools to convert to svg tiny. To get it working we had to save the images as svg tiny 1.1 + from illustrator.
