App icon disappears from menu when the phone is powered off and on

Hi,

I am developing an application for the 7650 using Symbian SDK 6.1.
I have successfully installed the app on the device. But when I power off the phone and power it back on again, the application icon is missing from the main menu. Has anyone observed this behaviour before?
If I reintall the application, it does so successfully (putting the icon back on the main menu), but warns that I am overwriting the existing version.

Any help apprciated.
