What is Application characteristics document

Hi everyone,
I have submitted my app to OVI Store but my app got rejected with following

"ISSUE #1: SUMMARY: Missing application characteristics document
EXPECTED RESULT: The application characteristic document is available and all the sections of the document must be filled in and questions are answered.
ACTUALT RESULT: Application characteristics document has not been provided"

I tried to find out but i dint get any link for this kind of document on OVI Stores site
Can anybody please suggest where i will get format of this document?

I think they mean the "declaration document" PDF, which you can get from the publisher Support control panel:

You may also have to provide, in addition, a flow chart.
