I think the title is pretty clear but just incase, How can I set the tile I create to show the small image by default (I know the user will still have the option to change size to medium but I want it to start as small).

That's not possible. By default when a tile is created its set to the Medium default size. Its up to the user to select the tile size. If you want your app to have good rating I would suggest to give all 3 sizes equal importance and create content for each size. There are metrics that show that an app with good tile implementation usually gets better ratings.
