I wonder what the file extension for sp-midi is. Is it the same as for midi files (*.midi)? In that case, why is there a special MIME type for sp-midi files?
File extension for all MIDI content, including SP-MIDI, is *.mid. MIME type is needed e.g. for MIDI engines to determine whether the file is GM1 MIDI-file or SP-MIDI -file. It is also needed in content protection issues.

How to distinguish midi from sp-midi files?

If both midi and sp-midi files have the same file extensions, how can one distinguish whether the a *.mid file contains midi or sp-midi so that one can assign the correct MIME content type?
SP-MIDI can be recognised from the Maximum Instantaneous Polyphony (MIP) message, which is placed to the beginning of the MIDI song. There's also tools that understand SP-MIDI, e.g. MIDIscale.
